    Please complete this form in its entirety, while making sure the provided information is correct and accurate, so that our ITAD Client Support team can guarantee the highest quality of services offered to your Clients. The filled in information will determine every step of the subsequent ITAD process, so please consider your answers thoroughly before submitting. It is mandatory for you to discuss every commercial aspect (e.g., service fees, deal type) determined via the form with your Client and to get their approval in advance.
